6 month PCP Supervised Diet?
How many of you had or have to do a 6 month PCP Supervised Diet? What are some of the things that the PCP/Nurse should be writing in my file so I make sure that the correct notes are being taken for my insurance (BCBS of MI)?
Any info or tips are helpful and appreciated!

I didn't have six months supervised diet...but I did have to supply records to show that i was fat for at least two years!...lol
There is a lot to be learned in the next six months, not just about losing weight, but about how incompetent people in the medical field can actually be!
KEEP COPIES OF EVERYTHING!...somehow, paperwork disappears and can cost you time having things re-done...keep a folder with proof of all appointments, tests and phone and fax numbers of EVERYONE you deal with through your doctor's office, benefits department, and insurance company!
Starting a food journal now will not only be a great habit to start, but if there ends up being any questions or "missing information" you will have your own journal to help fill in the blanks.
DOCUMENT YOUR STARTING POINT! Looking back now, i'm glad i took a "before" pic, but i never thought to take my starting measurements...I'd love to know now how many inches I've lost..In about a year, you will be looking back at the pictures and journals and probably barely recognize the life you changed forever!
